After contacting the Discipline Director(s), complete the following materials:

If applying for the Self-Advocacy discipline, please review the Qualifications sheet to see eligibility requirements.
Once a trainee is accepted to the CA-LEND program, final admission will be contingent on completion of all onboarding requirements.
